Changes to application forms accessed via Connect

We are improving the application and notification forms available on Connect[1] to make it easier for firms and individuals to submit high-quality applications.

Completing an application form is often the first interaction with us, and we want that experience to reflect the standards of a modern, data-led regulator.

Through extensive user research with firms and individuals, we identified key changes that would make our forms clearer, faster to complete and better aligned with the information we need to carry out our regulatory responsibilities. 

Transforming our Gateway authorisation forms

This work forms part of our wider Gateway Digital Transformation Programme, which aims to improve how we collect, process, and use data submitted through our regulatory gateway.

As part of this project, we are:

• Redesigning application and notification forms to make them more intuitive.
• Improving question structure to reduce avoidable queries.
• Ensuring we capture high quality regulatory data from the outset.

Please note that regulatory data and Reg[2]ul[2]at[3]ory d[4]ata submissions[5] will not be affected by these changes.

Gateway forms available to firms now

The following updated forms are now live and available for use. You can also submit feedback on any of them as part of our ongoing improvements:

• Form A: Apply to perform Controlled Functions, including Senior Management Functions.
• Form B: Withdraw an application to perform Controlled Functions, including Senior Management Functions.
• Form C: Used by firms to cease controlled functions.
• Firm Details Attestation Form: Update or attest to your firm details.
• Sensitive Business Names: Used when your firm wants to use a sensitive name or expression.
• Payment Services Directive (PSD) Agent forms: Used by firms to add, remove or amend details of a PSD agent.
• Waivers: Used to when your firm wants to amend or change a rule

Forms available for testing with a limited number of firms

• Application to Register as an Annex 1 Institution: Used by firms to register under the Money Laundering Regulations.
• Application to Register as an Annex 1 Individual: Used by firms to register an Individual under the Money Laundering Regulations.

We will continue adding more updated forms as they become available.

For all other forms not yet updated, you should continue to use Connect via My FCA[8].
